Open Scheduling

Establish with a Primary Care Physician by scheduling your initial appointment below.

To schedule your appointment:

1. Click on either the "New Patient Appointment" or "New Patient Appointment - Saturday and Evening appts" buttons below for a primary care appointment.
2. Review primary care physician availability and select a date and time.
   • If there's no availability on a specific day or time, you may scroll to the bottom of the page and select "Show more appointment times".
3. You will be prompted to log-in and schedule your appointment using MyChart or you can schedule the appointment as a 'guest'.
4. Follow the prompts to complete the necessary information in order to schedule your appointment.

If you prefer to schedule via phone, please call (805) 692-4650 and our Patient Outreach Coordinators can help with the selection of the new primary care provider.
