One of the key functions of Sansum Clinic's Breast Care Program and the Breast Care Alliance is the role of a Breast Care Navigator. A Breast Care Navigator works with the Breast Care Alliance's multidisciplinary team to design a comprehensive individual care plan for each patient based on his or her personal needs and medical history, and helps manage and coordinate doctor's office visits, diagnostic tests, and facilitates communication between physicians involved in the patient's care as well as the primary care physician.
Evelyn Schladweiler, RN, BSN, CBPN-IC is Sansum Clinic's breast care navigator. She connects our breast cancer patients with the many resources offered throughout Sansum Clinic and the Santa Barbara Breast Care Alliance for the best possible outcomes, from diagnosis through treatment and recovery. Evelyn follows every patient throughout every aspect of treatment and aids in dispelling fears associated with their diagnosis.
Evelyn is an excellent resource for information about breast cancer education and research and lifestyle changes or financial concerns. She collaborates with other healthcare providers with the goal of reducing barriers to care and facilitating the process for the patient and their loved ones. Evelyn is a source of empowerment for the patient by providing comprehensive information, compassionate support, and advocacy. She is also fluent in Spanish.
