Primary Duties (This list may not include all of the duties assigned.)
1. Acts as the primary medical contact for the Doctors’ Weight Management/HMR (Health Management Resources) meal replacement diet program.
2. Contributes nursing/medical perspective to patient education as part of a multidisciplinary team.
3. Participates in patient education to enhance patient safety and prevention of illness, reinforce and clarify meal replacement prescription, side effects of dieting, and program procedures.
4. Pre-screens patients by reviewing health history to determine contraindications for program participation.
5. Performs weekly patient evaluations through interview and physical assessment including vitals and lab tests.
6. Draws blood, processes and submits specimens to lab.
7. Reviews medical charts and lab data to provide timely intervention as necessary, to promote safety and diet compliance.
8. Responds to patients’ medical inquiries.
9. Communicates with physicians, health educators and other medical personnel regarding medical and behavioral plans to ensure effective and efficient patient care.
10. Collaborates with other team members to ensure efficient patient flow during medical monitoring times.
11. Documents all activities/interventions, patient/family responses, medication in EMR.
12. Maintains exam rooms for necessary supplies and materials. Ensures cleanliness.
13. Maintains strict patient confidentiality in compliance with HIPAA.
14. Must thoroughly understand and follow established policy and procedure with regard to basic patient care.
15. Participates in department quality improvement, clinic safety, infection control and hazardous materials programs and activities.
Hours/Week 36 Hours Per Week
Minimum Qualifications QUALIFICATIONS: Must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and abilities of personnel placed in similar or the same positions: Ability to read, write and communicate effectively in English. Ability to interact with patients, clinical, and administrative staff effectively on a daily basis. Must perform in an organized, accountable, professional manner. Must demonstrate proficiency in public speaking. Must be show proficiency in documenting in an electronic medical records system. Must be able to effectively make discrete observations and be able to receive and interpret pertinent details when interacting with patients. Must demonstrate decision-making and problem solving skills. Must be able to do math calculations on the spot. Must be able to multi-task with accuracy. Must be able to compare and identify differences in text (i.e. proofread and check calculations). Must have considerable motor coordination, manual dexterity, finger dexterity, eye-hand coordination and the ability to differentiate colors, shapes and shadings.
Must have knowledge of common safety hazards and precautions to establish/maintain a safe work environment. Knowledge of nursing principles, protocols, regulations and practices as pertinent to California to give and evaluate patient care. Must have the ability to react calmly and effectively in emergency situations.
EDUCATION/EXPERIENCE: Must have a valid license in the State of California as a Licensed Vocational Nurse with IV Therapy certification. Current CPR certification at time of hire. (ACLS and PALS may be required for specific departments. i.e. Pediatrics and Urgent Care). New graduates are welcome however a minimum of one (1) to two (2) years LVN experience in a clinic setting is preferred. Requires passing the RN/LVN and Clerical competency examination with a score of 85% on each.
