Your unique MyChart activation code can be generated in three ways:
- by calling the MyChart Response Team at (805) 898-3188
- by registering online using the form below
- by stopping in any of our sites in-person and requesting an activation code
MyChart requires that you provide an active Email account upon registration. This will help make sure that you receive notifications when new information is posted to MyChart. This is important since Sansum Clinic providers use MyChart to send certain test results and other messages.
