Understanding Your Medical Record Information

The medical record can also serve as a tool in educating health professionals and can provide a valuable source of data for medical research and quality improvement initiatives. The medical record may also be utilized by public health officials charged with improving the overall health of the community. Additionally, the medical record may be used as a source of data for facility planning and marketing.

Understanding what is in your record and how your health information is used helps you to ensure its accuracy, better understand who, what, when, where, and why others may access your health information and make more informed decisions when authorizing disclosure of that information to others.
